Starting a Program

New Programs with Pajaro Valley Unified School District

Step One: Obtain Sponsorship

Contact a PVUSD school, and talk to a principal about your program. If you have difficulty making contact, members at UCSC’s Educational Partnership Center may be able to help.

Step Two: Receive District Approval

Once you have sponsorship from a school principle, obtaining district approval just takes an email to PVUSD human resources. Your email should include a description of your program or service, information about the school(s) you will be working with, details on how many volunteers and how many youth participants you anticipate, and as much detail about the program structure and content as possible.

Step Three: Obtain Clearance for All Volunteers

Complete a Volunteer Form

Fill out an AUP Form

  • On the “District Computer Network User Information Form”, select “New User” and feel free to leave office address and office fax number blank.
  • Email to aida_alvarez@pvusd.net when completed

Show proof of a negative TB result

  •  A TB test result from within the last 4 years is necessary
If you have submitted a TB test result to UCSC in the last 4 years, please access it by:
  • Go to https://healthcenter.ucsc.edu

  • Scroll down and click the blue button that says “Health E-Messenger Log in here”

  • Enter your info to get into your account

  • On the left side, find the tab labeled “Medical Clearances”

  • If you’ve submitted a TB test result, you should see a field “TB Testing”

    • To access your submission, click “Update”

  • Download your uploaded form

  • Email this form to aida_alvarez@pvusd.net

If you need a TB test:
  • You’ll need to head to Watsonville, address is on the attached TB letter, no appointment necessary

  • You’ll have to bring the attached TB letter with you

  • You’ll need to return a second time in 48-72 hours to have your test read

  • Email your test results to aida_alvarez@pvusd.net

Visit a LiveScan facility and get fingerprinted

If you are in the Santa Cruz area:
  • Go to http://www.applicantservices.com/PVUVOL to make an appointment

    • Enter your email to get an access code

    • Fill out the online fingerprint form

  • When you search for LiveScan location, search “Watsonville, CA” and PVUSD should be the one option that pops up

    • Select an appointment time

  • You will receive an email with a 7-digit “Payment ID” code

    • Take this with you to your appointment

If you are in California but outside the area:
  • Go to http://www.applicantservices.com/PVUVOL to make an appointment

    • Enter your email to get an access code

    • Fill out the online fingerprint form

  • Search for a location near you and select an appointment time

  • Take the attached form “Request for Live Scan” to any livescan location

  • Get a receipt, and we will reimburse you with program funds for the charge

If you are out of state:
  • Visit http://www.applicantservices.com/PVUVOL

  • Complete the information as requested

  • When asked to input your location, you will not see any locations outside of California (not permitted by Cal-DOJ).  But you will be presented with the Card Scan option.  Select the option for “Card Scan”

  • You will be provided information to your local law enforcement agency that can take your fingerprints on cards (just as a reference, please verify with the agency)

  • Visit your local law enforcement agency and make a request to be fingerprinted on a hard card

  • Secure two copies of the hard card and mail to the following address:

    • Capital Live Scan

    • ATTN: AS.COM Inc. Card Processing Center

    • 5706 Broadway

    • Sacramento, California 95820

  • Be sure to also include the Request Form with the Payment ID on it.  Also, write the Payment ID on the top area of the cards (any blank area is fine)
